David Hazlewood, 61, of McAlester, died Thursday, Jan. 10, 2013, at his McAlester residence.
Services are set for 2 p.m. Tuesday at Bishop Chapel of Memories in McAlester, with W. Lee “Dub” Brown officiating.
Internment will be at Oak Hill Cemetery. Arrangements are under the direction of Bishop Funeral Service.
He was born Oct. 28, 1951 in McAlester to Vestal and Aurilla Jones Hazlewood. He attended McAlester Public Schools and graduated from McAlester High School in 1969. He then attended Oklahoma State University in Stillwater, where he received his Bachelor Degree in 1974. He then attended the University of Oklahoma where he received a Degree of Pharmancy in 1979. He worked for the Indian Heath Services and was director of pharmacy in Talihina. After retirement he worked for a time at Head’s Pharmacy in Stigler. He was a member of the First Baptist Church of McAlester.
Survivors include a son, Shawn Hazlewood, of Norman; a daughter and son-in-law, Katie and Chris Thompson, of Oklahoma City; a sister, Diane Malicoat, of McAlester; father, Vestal Hazlewood, of McAlester; a nephew, Dennis Malicoat, of McAlester; a great-neice, Hattie Belle Malicoat, of Durant; aunts Mellanie Shuman, of McAlester; Lillian Wood, of McAlester, and Ann Anderson, of Muskogee. He was preceded in death by his nother, Aurilla Hazlewood.
Active pallbearers are Jerry Shackelford, Richard Carswell, George Zellmer, Larry Owens, Jim Ray Thompson and Jimmie Williams.
Honorary pallbearers are Richard Odle, Russell Martin, Tom Craig, Ronnie McGee, Gary Nichols, Gary Manos and the McAlester High School Class of 1969, classmates and friends.
